Healthy Bergen, Powered By The Community Health Improvement Partnership (CHIP), Launches 2025 Community Health Needs Assessment, Seeking Public Input Assessment informs implementation plan for Bergen County for the next three years

Hackensack, N.J. (May 2, 2025) — Healthy Bergen, powered by the Community Health Improvement Partnership (CHIP), in collaboration with all seven non-profit hospital systems within Bergen County, recently launched the 2025 Community Health Needs Assessment (CHNA). Participating hospitals include Englewood Health, Bergen New Bridge Medical Center, The Valley Hospital, Holy Name, Hackensack Meridian Pascack Valley Medical Center, Hackensack Meridian Health, and Christian Health.
The CHNA Survey, conducted every three years, helps local health organizations and community leaders identify and prioritize top health concerns, develop targeted strategies to address community health needs and gaps in coverage, secure funding and resources to implement programs that support better health outcomes and strengthen partnerships among local organizations to improve health services. Based on the assessment results, Healthy Bergen will develop a comprehensive strategy to address key concerns by connecting people to available resources and services in Bergen County. The plan will focus on healthy minds, healthy bodies, and healthy communities, with a focus on the issues highlighted throughout the assessment. “The CHNA Survey helps us to identify and address the key issues in the Bergen County community, but we need robust community participation to gather the most accurate results,” said Lisa Pinto, CHIP Coordinator. “Understanding the needs and challenges of the entire Bergen County community is essential to our ability to fulfill Health Bergen’s mission and help connect people to the programs and resources they need to live their healthiest lives.”
